Why investigation quality matters

ICAM or 5 Whys?
Different incidents call for different methods.
ICAM (Incident Cause Analysis Method) does not focus on individual actions alone. It gives you a consistent, evidence-based framework for investigations and supports more robust corrective actions that reduce the likelihood of recurrence. It is widely used across high-risk industries for incidents ranging from near misses to serious and fatal events.
Training
- ICAM Lead Investigator: for HSE, operations and maintenance supervisors who lead or facilitate investigations. Delivered face to face at our training centre in Dubbo, in partnership with Safety Wise (RTO 21871), with an optional assessment for the nationally recognised unit BSBWHS515 Lead initial response to and investigate WHS incidents.
